Medical Forms

• **New** Medical Form / Consent Form for Adults and Scouts

Instead of Class1, Class2, and Consent to Treat, New forms are Parts A, B, C.

• Part A: Annual Health and Medical Record – Required for all Campouts.
• Part B: Annual Doctors Physical Examination – Required for Campouts over 72 Hours.
• Part C: Annual Informed Consent and Hold Harmless/Release Agreement – Required for all campouts and must be notarized. This also includes a talent release form.
• Everyone is required to get the yearly physical for camping more than 72 hours. This includes scouts and scouters. Not just the over 40 group.

All forms are required to be updated annually.
